This summary explains how information submitted to the TORCH Texas Rural Hospital Intelligence subscription waitlist is used.

What the form collects

Name, work email, title, organization, organization type, stated priorities, preferred briefing format, optional comments, communication choices and—when selected—the hospital's public CMS Certification Number.

Why it is used

To confirm the hospital or organization, manage subscription onboarding, communicate about the monthly briefing and improve service delivery. Vercel supports site hosting, Resend delivers request and confirmation emails, and Attio supports customer-relationship management and follow-up.

Hospital search on this device

If you choose a hospital, its public name, city and CMS identifier are kept in your browser for the current session so the request form can be prefilled. That selection is sent to Talon only if you submit the form.

What not to submit

Do not submit patient information, claims data, passwords, credentials, confidential hospital records or other sensitive information through the public form.

Your communication choices

The required consent applies to requested subscription and onboarding communication. Broader Talon research and product updates require a separate optional choice. You may unsubscribe from those communications at any time.

Questions or changes

You may ask to correct or remove your waitlist information by replying to a Talon communication connected to your request. This summary may be updated as the service, its delivery processes or its supporting providers change.
